There's a frustrating problem in rehab: the times you most need to rebuild muscle, after surgery, with a painful joint, early in an injury, are exactly the times you can't lift heavy enough to build it. Blood flow restriction training solves that. By briefly and safely limiting blood flow to a limb while you exercise, we can produce strength and muscle gains normally reserved for heavy lifting, using very light loads. What blood flow restriction training is
Blood flow restriction training, BFR, sometimes called occlusion training, involves placing an inflatable cuff around the top of a limb (arm or leg) and inflating it to a set pressure while you perform light resistance exercise. The cuff partially restricts the arterial flow in and significantly restricts the venous flow out, so blood pools in the working muscle. You then exercise with light weights or bodyweight, typically at around 20–30% of what you could normally lift, for higher repetitions.
The remarkable part is the result: that combination tricks the muscle into responding as though it were training heavy.
How it works
When you restrict blood flow and exercise, the muscle quickly runs low on oxygen and metabolites build up. That metabolic stress does several things:
- It recruits your bigger, fast-twitch muscle fibres early. Normally these only switch on under heavy load; the low-oxygen environment brings them into play at light loads.
- It drives muscle protein synthesis. The metabolic stress and cell swelling trigger the same growth signals as heavy lifting.
- It builds strength and size at loads a healing joint can tolerate. You get the adaptation without the mechanical strain that heavy weights put through a recovering knee, shoulder or tendon.
In short, BFR lets us separate the training stimulus from the heavy load, so we can strengthen a muscle that isn't ready to be loaded heavily yet.

What we use it for
BFR is one of the most useful tools we have for rebuilding muscle when heavy loading isn't an option. We use it for:
- After surgery, especially post-operative rehabilitation where muscle wastes fast, such as the quadriceps after ACL reconstruction or knee surgery. BFR helps limit that wasting and rebuild strength early, when heavy loading isn't safe.
- Painful joints and tendons, arthritic knees and hips, and tendinopathies, where you can build strength without provoking the joint or tendon with heavy weights.
- Early-stage injury rehab, maintaining and building muscle while a tissue is still healing.
- Deconditioning and reduced capacity, rebuilding after a period of rest, illness or immobilisation.
- Athletes managing load, adding a strength stimulus without piling on more heavy training on a fatigued or niggling area.
Personalised pressure with the VALD AirBands
This is where the how matters as much as the what, and where a lot of BFR is done badly. The right cuff pressure is individual: too little does nothing, and too much stops being "restriction" and becomes full occlusion, which is uncomfortable and riskier. Elastic wraps and one-pressure-fits-all cuffs are guesswork.
We use the VALD AirBands, a pneumatic system that works by measuring each person's limb occlusion pressure, the exact pressure at which blood flow to that limb stops, and then setting the training pressure to a safe, effective percentage of it (typically 40–80%). It's personalised to you, to that limb, on that day, and it's tracked. That precision is what makes BFR both safe and worthwhile: you get the right dose, measured, rather than a rough approximation.
How it fits into your rehab
BFR isn't a standalone treatment, it's a way of delivering strengthening within your physiotherapy programme. In a session we fit and set the AirBands to your measured pressure, then take you through specific light-load exercises for the target muscle, and build it into your wider rehab and home plan. As you heal and your capacity returns, we progressively reintroduce heavier loading and phase the BFR out. It's a bridge, a way to keep building strength through the phase when heavy lifting isn't possible yet, not a permanent substitute for it.
What the evidence shows
We'll give you the honest picture, and here it's genuinely encouraging: the evidence is strong. Multiple studies and reviews show that low-load training with blood flow restriction produces gains in muscle strength and size comparable to conventional heavy resistance training, often within a few weeks. In rehabilitation specifically, including after ACL reconstruction, BFR has been shown to help preserve and rebuild quadriceps strength and size, and reduce pain, under loads a recovering joint can handle, with results approaching those of heavy training.
The important caveats: technique and pressure matter enormously, the benefits and the safety both depend on personalised, correctly set pressure, which is exactly why we use a measured system rather than elastic bands. And BFR is a tool within a good rehab programme, not a magic bullet on its own. Used properly, it's one of the better-supported additions to modern rehabilitation.
Who it's for, and who it's not
BFR suits people rebuilding strength when heavy loading isn't possible or advisable, after surgery or injury, or with a painful joint or tendon. Because it involves restricting blood flow, it isn't suitable for everyone. Tell us before treatment if you have a history of blood clots (DVT or PE) or a clotting disorder, cardiovascular disease or uncontrolled high blood pressure, are pregnant, have sickle cell trait or disease, significant varicose veins, lymphoedema, or a cancer or vascular condition affecting the limb. We screen everyone thoroughly first and will tell you honestly if it isn't appropriate for you.
