Most rehab focuses on the muscle. The NEUBIE focuses on the nervous system that controls it, and that changes what's possible. It's a next-generation neuromuscular device we use within physiotherapy at Hanbury Health, and we're one of only a handful of clinics in the UK to offer it. If you've plateaued in recovery, can't seem to "switch a muscle back on" after surgery, or keep hitting the same wall with an old injury, this is often the missing piece. Here's what it is, how it's different, and how we use it.

What the NEUBIE is

The NEUBIE, short for Neuro-Bio-Electric Stimulator, is a neuromuscular electrical stimulation device made by NeuFit, cleared by the US FDA and used in clinics, universities and professional sports teams. Where it differs from a normal muscle stimulator is the current it uses: a pulsed direct current (DC) rather than the alternating current (AC) found in conventional NMES and TENS machines. That single difference is the reason it behaves so differently in a rehab setting.

We use it as a tool within physiotherapy, applied and supervised by a physiotherapist as part of an assessment-led treatment plan.

How it's different from a standard muscle stimulator

If you've had electrical stimulation before, pads stuck on while a machine twitches the muscle and you lie there, that's conventional NMES or TENS. The NEUBIE is a different proposition in three ways:

  • Direct current, not alternating. DC penetrates differently and, importantly, lets us stimulate through a fuller range without the sharp, clamping contraction AC produces. That makes it far more comfortable to combine with movement.
  • Used during active movement, not passively. This is the key. Instead of lying still while a muscle twitches, you actively move, squats, step-downs, reaching, controlled eccentrics, while the current is applied. We're training the nervous system to fire the right muscles at the right time, under load, in real movement.
  • It targets the nervous system, not just the muscle. After injury or surgery, the nervous system often "guards", it inhibits muscles and limits movement to protect you. That protective wiring is frequently what's actually holding recovery back. The NEUBIE gives us a direct way to work with it: to down-regulate that guarding and re-activate muscles the brain has learned to switch off.

In short: standard stimulation exercises a muscle for you. The NEUBIE helps us re-train how your nervous system controls that muscle while you move.

What we use it for

Our focus is musculoskeletal, sports and post-surgical rehabilitation. In practice, the NEUBIE is especially useful for:

  • Post-operative muscle re-activation. After knee surgery (ACL reconstruction, for example), the quads, and the VMO in particular, often won't fire properly, however hard you try. The NEUBIE is one of the most effective tools we've used for waking those muscles back up early in post-operative rehabilitation.
  • Stubborn, plateaued injuries. Old hamstring, calf, groin or shoulder problems that keep recurring, where the issue is motor control and inhibition rather than tissue damage.
  • Tendon and chronic pain problems. As part of a loading programme for tendinopathies, and as a way to modulate pain so you can move and load more comfortably.
  • Strength when loading is limited. When an injury, surgery or pain means you can't yet lift heavy, the NEUBIE lets us generate a strong training stimulus at lower external load.
  • Sports performance and recovery. For activation before training and recovery afterwards, as part of a sports rehabilitation plan.

The NEUBIE is also used elsewhere for neurological rehabilitation (stroke, MS and similar conditions). That's a specialist area where results vary considerably; our focus here is musculoskeletal, sports and post-surgical care, and we'll always be clear about what's realistic for your situation.

How it fits into a physiotherapy session

The NEUBIE isn't a standalone treatment you book instead of physio, it's a tool we use within it, following the structured NeuFit approach:

  1. Assess and map. We assess your movement and use the device to scan for areas where the nervous system is limiting you, where muscles are inhibited or guarding is restricting movement.
  2. Stimulate while you move. We apply the current to the targeted areas while guiding you through specific movements and exercises, often with a focus on the lowering (eccentric) phase, to re-educate how those muscles fire.
  3. Reassess. We re-test the movement to see what's changed, and fold the gains into your ongoing rehab and home programme so they hold.

You'll feel a strong but manageable sensation, and we adjust the intensity to you throughout. Many people notice a change in a movement or a muscle "switching on" within the first session, though lasting change comes from repeating the process and doing the rehab around it.

We combine the current with guided movement and eccentric exercise.

Why it's different from standard physiotherapy

To be clear: excellent physiotherapy, assessment, hands-on treatment, and a well-designed exercise programme, is the foundation, and the NEUBIE doesn't replace any of it. What it adds is a direct line to the nervous system that we don't otherwise have. It lets us address the neurological side of an injury, the guarding, inhibition and faulty motor patterns, that hands-on work and exercise alone can be slower to shift. That's why it so often helps where progress has stalled: we're not just strengthening the muscle, we're changing how your body is allowed to use it.

What the evidence shows

We'll be straight about this, because the internet isn't. The NEUBIE is FDA-cleared, taught in over twenty US university programmes, and used by many professional sports teams, but the published, independent evidence base is still developing, and much of what's promoted is case studies and manufacturer data. We treat the more dramatic claims with appropriate caution.

That said, the mechanism is sound, and the peer-reviewed evidence is beginning to catch up. In a 2025 randomised controlled trial, direct-current stimulation of the NEUBIE type significantly outperformed conventional TENS for diabetic peripheral neuropathy, improving pain, sensation and nerve-related measures where TENS showed no significant change. Our own experience mirrors the pattern the research points to: as an adjunct within skilled physiotherapy, it helps us get results, particularly in muscle re-activation and stubborn cases, faster than we would without it. We use it because it works in our hands, and we're honest that it's a complement to good rehab, not a miracle.

Who it's for, and who it's not

The NEUBIE suits people who are in rehab for a musculoskeletal or sports injury, recovering from orthopaedic surgery, or stuck on a problem that hasn't responded to standard approaches. As with any electrical stimulation, it isn't suitable for everyone. Tell us before treatment if you have a pacemaker or any implanted electronic device, are pregnant, have epilepsy, have a known blood clot (DVT), have cancer in the area to be treated, or have broken or infected skin over the treatment site. We assess everyone first and will tell you honestly if it isn't appropriate for you.

One of the few clinics in the UK with a NEUBIE

The NEUBIE is well established in the US but still rare in the UK.
